Overview

Join the Coborn’s Pharmacy Team as our Centralized Clinical Pharmacist. We seek a self-motivated, patient-focused professional to lead and manage Medication Therapy Management (MTM) for our Coborn locations.

Along with managing MTM, you will be responsible for remarkable patient care! Through telephonic calls, you will help patients understand and manage their medications to ensure they are working as effectively as possible. After the call is completed, you will actively partner with the patient’s Physician and applicable Pharmacy location to discuss changes in care and adjustments to the medication therapy for the patient following each encounter.

Additional Duties Include:

  • Lead and manage the day-to-day activities that support the HealthPartners Medication Therapy Management (MTM) program for all Coborn’s, Inc. Pharmacy locations. This includes providing centralized MTM support to all patients referred to Coborn’s, Inc. locations through the MTM program.
  • Provide remarkable one-on-one assistance to patients over the phone. This includes reviewing current medications, assessing their effectiveness, answering patient questions, and following up with the Physician and Pharmacy staff to coordinate ongoing care.
  • Provide remarkable patient care through telephonic calls with assigned patients.
  • Collaborate with the patient’s Physician and applicable Coborn’s Pharmacy Staff on the coordination of care for each patient. This position will discuss changes needed in care or adjustments to the patient’s medication therapy after each encounter.
  • Document patient encounters on electronic health records using clear, concise, professional language. Ensure that patient-facing documentation is in language that is easy to understand.
  • Consistently interact with patients staff and others in a professional, respectful, friendly, and engaging manner consistent with a retail pharmacy environment.
Requirements

EDUCATION & EXPERIENCE

  • Pharmacy B.S. and/or PharmD degree(s) from an accredited College of Pharmacy.
  • Proven leadership skills and business acumen are preferred but not mandatory.
  • Must be self-motivated, energetic, and customer service-oriented.
