JOB REQUIREMENTS: JOB PURPOSE AND REPORTING STRUCTURE: Under thesupervision of the Director of Pharmacy, the Clinical Pharmacist is amember of the clinical interdisciplinary care team. The ClinicalPharmacist delivers pharmaceutical care in accordance with the policiesand procedures of Outreach Community Health Centers. ESSENTIAL DUTIESAND RESPONSIBILITIES: Ensure that medications reach the patient in thecorrect dose, form, vehicle and at the correct time. This includestablets, liquids, drops, capsules, ointments, injections, inhalers,creams, beads, liposomes, continuous delivery systems and other formsthat medications may take. Take responsibility for the accurate,appropriate and timely use of medications administered to patients bystaff in the clinic setting. Take responsibility for the accurate,appropriate and timely dispensing of medications for patients uponpresentation of valid prescriptions in the outpatient pharmacy setting.Supervise and monitor the work of pharmacy technicians in compliancewith State and Federal Requirements. Teach and respond to queries ofpatients and their family\'s providers and other staff regardingaccurate, appropriate and timely use of medications includingindications, dosing regimens and side effects. Stay current on newinformation and developments and educational material in Pharmacy toserve patients, providers and other staff. Collaborate with providersand other health care staff including Community Service, BehavioralHealth, Primary Care and other Medical Specialties to deliver safe,effective and economic medication use and delivery. Counsel patients ondosage, administration, intended effects and unintended side effects ofmedications. Monitor the stages of medication therapy to improvereporting of side effects and challenges with overall safe delivery.Give special consideration to patients requiring multiple medications,complex regimens of therapy, and barriers to compliance with the goalsof therapy. Participate and complete special assignments and projects asassigned by the Director of Pharmacy. Assists in inventory control andproper storage of medication. Maintain professional standing andlicensure in accordance with the State of Wisconsin Department of Safetyand Professional Services. Maintain technical competency and skillsrequired for employment as a pharmacist. Complete required documentationon a timely basis.
